Re: General Sessions Sentence Sheet, SCCA/217
ORDER
Pursuant to the provisions of S.C. Const. Art. V § 4,
IT IS ORDERED that Form SCCA/217, General Sessions Sentence Sheet, bearing a revision date of 07/2008, is approved for use in the Circuit Courts of South Carolina and replaces SCCA 217, bearing a revision date of 10/2007.
The form has been amended to add a line for the SC Bar number to identify the attorney for defendant. A line for the $5.00 surcharge to fund training at the South Carolina Criminal Justice Academy, pursuant to § 90.11 of the Temporary Provisions of the 2008-2009 Appropriations Act, has also been added. Pursuant to § 90.11, the $5 surcharge shall go into effect on July 1, 2008. This form shall be available on July 1, 2008 on the South Carolina Judicial Department website at www.sccourts.org under the ‘Forms’ link.
